Ticket to Work Participant Forms

Below are various forms needed as participants in the Ticket to Work Program. 

New Employment Packet

Once your ticket is approved, you are to use these documents and resources below to abide by the SSA Ticket to Work program rules & guidelines. Definitions for usage are provided.

Once you receive employment, stop employment or any changes in employment (earnings, increase or decrease in work hours), SSA needs to know this information. Providing this information will help to reduce overpayments.

Once you start work, you must keep track of monthly GROSS earnings to help you and your EN know where you are (tracking) in the Ticket to Work program. There are three phases, Trial Work Period, Extended Period of Disability & Reinstatement of Benefits. Every phase listed has different rules and this Monthly Earnings Calendar will accurately advise you in which phase you are currently in. Enter your gross wages for every paycheck EVERY MONTH using this calendar & submit to EN EVERY 3 MONTHS for review. In addition submit copies of your paystubs every month to your EN and to SSA. 

When you start working with a Ticket Program service provider (EN), you'll develop a plan that describes your work goals and the help you can expect from your service provider to reach those goals. This plan is a written agreement that details the responsibilities of you and your service provider. Social Security will review your progress (a Timely Progress Review) to make sure you're following your plan and are on track to reach your goals.

For SSDI & SSI beneficiaries it is MANDATORY to report your monthly earnings to your service provider (EN) and your local SSA office. They use this information to determine if you receive your monthly cash benefits. Use this guideline for learning your reporting procedure. Report your earnings by the end of each month worked.

Under a Work Review, Social Security looks at earnings to determine if an individual is eligible for monthly benefits. If you continue to work while receiving Social Security benefits, then you need to keep track of ALL of your pay stubs and monthly earnings. You will receive this review usually at the beginning when you start work & at the 10th month of your employment. This is a standard routine procedure to determine if you are still eligible for your Disability benefits.
